Cardiac sarcoidosis is a complex, multi-system disease marked by non-caseating granulomas and fibrosis in any organ, including the heart. It affects about 5% of individuals, posing diagnostic challenges. In the heart, it can disrupt the conduction system, leading to heart blocks or bundle branch blocks. Additionally, it may cause atrial arrhythmias and ventricular tachycardia, potentially resulting in sudden death. Cardiac sarcoidosis can mimic arrhythmogenic right ventricular cardiomyopathy (ARVC), but tends to involve the septum more frequently. Unlike ARVC, sarcoidosis is typically not familial. Septal scars and arrhythmias might indicate sarcoidosis over ARVC.
