The most appropriate next step is to start empiric oral antibiotics and reevaluate. This is an example of a superficial CIED pocket infection. This is the diagram that shows you how to approach it. It's an early superficial site infection such as erythema and or stitch abscess localized to the superficial aspect of the wound. Within the first 30 days of device placement, without fever, without systemic toxicity, a course of oral antibiotics is indicated. If therapy fails and there's evidence of more systemic infection, then of course you have to think about taking it out. And that means that you're removing the CIED, the implantable electronic device, cardiac implantable electronic device, as well as the generator and all transvenous leads.
