Intersection of EP and HF: Making GDMT Happen: Operationalizing HF Risk Factor Modification and Uptitration in EP Centered Care
Description
Dr. Harriette Van Spall, cardiologist and clinical trialist at McMaster University, discusses how to implement heart failure guideline-directed medical therapy in patients at risk for or living with atrial fibrillation. She explains how heart failure and AFib share common risk factors under the umbrella of cardiovascular-kidney-metabolic syndrome, with adiposity as a key driver. She synthesizes evidence from pivotal HFpEF trials, including SGLT2 inhibitors, MRAs, GLP-1 receptor agonists, ARBs, and ARNIs, and shows how these therapies benefit patients with or without AFib. She closes with a look at how to apply therapies across heart failure stages and highlights ongoing trials testing new drugs and pacing strategies in HFpEF.

Learning Objectives
  • Describe the shared risk factors and mechanisms linking heart failure and atrial fibrillation, including the role of adiposity.
  • Recognize the challenges of diagnosing HFpEF in patients with atrial fibrillation and the tools available to confirm it.
  • Summarize the evidence for SGLT2 inhibitors, MRAs, GLP-1 receptor agonists, ARBs, and ARNIs in HFpEF, including their effects in patients with AFib.
  • Apply guideline-directed medical therapy across heart failure stages, including in patients with chronic kidney disease or obesity.
  • Identify emerging trials evaluating new drugs and pacing strategies in HFpEF.
