Intersection of EP and HF: Why HF and Arrhythmias Travel Together: Mechanistic Links Between AF, Cardiomyopathy, and Outcomes
Description
In this webinar, Dr. Elaine Wan, Associate Professor and Director of Electrophysiology Research at Columbia University, explains why heart failure and arrhythmias so often go together. She walks through the bidirectional cycle linking atrial fibrillation and cardiomyopathy, driven by atrial stretch, fibrosis, and neurohormonal activation, and shows how having both conditions sharply worsens prognosis. She reviews the evidence for early rhythm control, including catheter ablation outcomes from the CASTLE-AF trial, and discusses how implantable multi-sensor algorithms can flag worsening heart failure weeks before decompensation. She closes with a call for heart failure specialists, electrophysiologists, and cardiologists to work together to improve outcomes for these patients.

Learning Objectives
  • Describe the mechanisms that link atrial fibrillation and heart failure in a bidirectional cycle, including atrial stretch, fibrosis, and neurohormonal activation.
  • Recognize the increased mortality, hospitalization, and stroke risk in patients with both atrial fibrillation and cardiomyopathy.
  • Summarize the evidence supporting early rhythm control, including catheter ablation, in patients with heart failure and atrial fibrillation.
  • Explain how implantable multi-sensor algorithms can provide early warning of heart failure decompensation.
