Intersection of EP and HF: Making GDMT Happen: Operationalizing HF Risk Factor Modification and Uptitration in EP Centered Care Part 2
Description
In this webinar, Dr. Carolyn Lam, heart failure cardiologist, offers a heart failure perspective on patients living with both atrial fibrillation and HFpEF. Using a clinical case, she shows how EPs and heart failure specialists often see the same patient through different lenses, and how frequently the two conditions overlap. She reviews the latest ESC and American guideline recommendations for managing AF in heart failure, the evidence for early rhythm control and catheter ablation, and why the data in HFpEF remain limited. She proposes viewing AF and heart failure as conditions with a "common soil" of inflammation, metabolic disease, and fibrosis, and highlights how foundational medical therapies, including SGLT2 inhibitors, MRAs, and GLP-1 receptor agonists, can treat both. She closes with a call for EP and heart failure teams to bridge their two cultures in service of one patient.

Learning Objectives
  • Recognize how often atrial fibrillation and HFpEF overlap, including occult HFpEF in patients referred for AF ablation.
  • Summarize current ESC and American guideline recommendations for managing atrial fibrillation in patients with heart failure.
  • Compare the evidence for catheter ablation in HFrEF versus HFpEF and identify where clinical equipoise remains.
  • Describe the "common soil" model linking AF and heart failure through shared inflammatory, metabolic, and fibrotic pathways.
  • Identify foundational medical therapies, including SGLT2 inhibitors, MRAs, and GLP-1 receptor agonists, that may reduce both heart failure events and AF.
