Intersection of EP and HF: From Signs to Signals: HF Diagnostics and Device‑Based Monitoring to Detect Early Decompensation
Description
In this talk, Dr. Marat Fudim, a heart failure cardiologist at Duke University, explores the overlap between heart failure and electrophysiology. He explains why EPs should screen for underlying heart failure, especially HFpEF in patients with AFib, and reviews therapies any clinician can start. He also shows how remote monitoring with implanted devices and sensors can detect decompensation early. He shares Duke data showing fewer hospitalizations and lower costs, and he makes the case for closer collaboration between EP and heart failure teams.

Learning Objectives
  • Recognize the clinical overlap between atrial fibrillation and heart failure, including the high prevalence of HFpEF in AFib patients.
  • Use simple diagnostic tools, such as NT-proBNP and the H2FPEF score, to identify underlying heart failure in EP patients.
  • Identify guideline-directed therapies, such as SGLT2 inhibitors, MRAs, and ARNIs, that EPs can start in patients with heart failure.
  • Describe how device-based remote monitoring, including CIEDs, PA pressure sensors, and multi-sensor algorithms, can detect early decompensation before symptoms or weight gain appear.
  • Discuss the clinical and financial benefits of collaborative remote monitoring programs between EP and heart failure teams.
