Video Summary
Heart failure and atrial fibrillation are tightly linked through shared cardiometabolic risks such as obesity, hypertension, diabetes, dyslipidemia, aging, and smoking. These factors drive inflammation, fibrosis, remodeling, and atrial arrhythmias, making AF both a consequence and a marker of more advanced heart failure, especially HFpEF. AF in heart failure is associated with higher mortality, more readmissions, longer hospital stays, and greater costs.

Diagnosis of HFpEF is often missed in AF, so objective testing—especially invasive hemodynamics, exercise echo, and biomarker interpretation mindful of rhythm and obesity—is important.

Guideline-directed therapies with evidence in HFpEF include SGLT2 inhibitors, non-steroidal MRAs like finerenone, and in obese patients GLP-1 receptor agonists such as semaglutide and tirzepatide. Lifestyle measures—diet, exercise, sleep, smoking cessation, blood pressure, glycemic, and weight control—are essential for prevention and treatment.
